Social prescribing is a way of connecting people to non-medical support and resources within their community to improve their health and wellbeing. Springfield Medical Centre has a dedicated Community Connector who can support you with non-medical, lifestyle and social needs. This could be finding out what support is out there in your community and choose from a menu of options tailored to you. This could be connecting to social and creative activities, engage in healthy lifestyle behaviours such as physical activity and moving more, connecting to communities of identity and experience, learning a new skill or prepare for employment at your pace, volunteering and much more to improve your health and wellbeing.
